Automatic medical-history update reminders

Teamio can automatically remind patients to update their medical history on a set schedule — by text, email, or both — and the reminder rides along with your normal appointment communications. Once it’s on, patients receive the reminders without staff sending anything.

Teamio configures these reminders for you. When you ask your Teamio contact to enable them, they open the Medical History Update Reminders settings from the Settings icon (the gear ⚙️) next to Create New Form and dial in the values below with you.

These are the values Teamio sets up with you:

SettingWhat it does
Enable medical-history update remindersThe master on/off toggle.
How often should patients update their medical history? The interval — a number plus month(s) or year(s). This is the clock between updates.
How should we notify them? The channel: SMS, Email, or Both.
Wiki SMS TemplateThe wiki page used as the SMS message body (required when SMS is on).
Wiki Email TemplateThe wiki page used as the email body (required when Email is on).
Form(s) that start the update clockThe medical-history form(s) whose last submission starts the interval — when that much time has passed, the patient becomes due.

Once the values are saved, the reminders apply automatically.

When a patient’s last medical-history submission is older than the configured interval, Teamio considers them due and sends the reminder on your chosen channel(s), using the wiki template(s) you picked. The reminder typically rides along with appointment confirmations rather than as a separate blast. The patient taps the link, verifies, and submits an updated MED_HX_UPDATE form — which refreshes the chart per its field mappings.

  • Keep the trigger form mapped. The reminder only adds value if the update form writes back to the chart — confirm its controls have Save to Open Dental Field mappings.
  • Write the template once, reuse it. The SMS and email bodies are ordinary wiki pages, so you edit the wording in the Wiki without touching these settings.
