Creating a form with AI

Tell Teamio what you want in plain English and it builds the form for you. The draft opens in the form builder unsaved, so you review and tweak everything before it goes live — nothing is created until you click Save.

On the Forms page, click Create with AI. A window opens where you describe the form.

The Start from dropdown lets you base the new form on one you already have:

  • A blank form (start from scratch) — the default. Describe the whole form.
  • Pick an existing form — AI starts from that form and applies the changes you describe, then opens it as a new draft. Your original form is never touched.

Type what the form should collect — in normal language. Be specific about the fields you want. For example:

Click the microphone to dictate your description out loud — the words appear in the box as you speak. Click it again to stop, then edit if needed.

Click Generate. In a few seconds the draft opens in the form builder. From here it’s a normal form:

  • Add, remove, and rearrange fields.
  • Set required fields, options, and conditional visibility rules.
  • Map fields to Open Dental — common patient fields (name, date of birth, phone, email, address) are mapped automatically; set the rest yourself.

Teamio may show a short note if it adjusted anything while building (for example, simplifying a field it wasn’t sure about) — give the draft a quick once-over.

The form is not saved yet. Give it a Form Name and Form Type, make any edits, and click Save Form. Until you do, nothing is stored.

  • It made too much or too little. Just edit it in the builder, or tweak your description and generate again — you’re always reviewing a draft, never overwriting a live form.
  • I want to change a form I already have. Use Editing a form with AI instead — it edits in place and shows you exactly what will change before applying.
  • The fields aren’t mapped to Open Dental. Only standard patient fields map automatically. Map the rest in Configuring a control.
