Form controls reference

This is the complete reference for every control you can drop into a form. There are exactly 20 control types in the Add Control to your Section sidebar; each entry shows a name and a one-line description. A control is added inside a section via Add Control, then tuned in its Configuration panel.

The settings below are in addition to the universal fields (Unique ID, Name, Label, Sub-label, Placeholder, Default Value, Save to Open Dental Field, Container Size, Highlight for Alert, Validation, Visibility) that every control shares — those are documented in Configuring a control. The layout controls (Label, Text Block, Button, Empty Block, Page Break) are the exception — they hold no patient answer, so they carry no Validation Rules at all.

Input Field — a single line of text (names, email, phone, etc.).

  • Input type — text (normal), password (masked), or hidden (stored but not shown). Use text for almost everything.
  • Pairs with the Is Email, Is Phone Number, Is Social Security Number, or Regex validation rules to enforce a format.

Number Input Field — numeric entry only.

  • Is decimal — off = whole numbers only; on = decimals allowed.
  • Decimal places — how many digits after the decimal point.
  • Use min / max validation rules to bound the value.

Text Field — a multi-line text box for longer answers (notes, history). It scrolls beyond its visible height.

Dropdown — pick from a list.

  • Data mode — Normal - Pre-Config List Items (you type the options) or API - List Items from your own API (options load from a URL).
  • Allow multiple — single-select vs multi-select.
  • Items (list mode) — each option has a value (stored) and text (shown).
  • API fields (api mode) — the URL plus the keys that hold the value and the display text.

Checkbox List — choose several from a list (multi-select).

  • Items — value + text per option.
  • Display mode — Line by Line (line, stacked one per line), Next to each others (next, inline), or Stay on each sides in a row (Left - Right) (bothSide).
  • Position — Left / Center / Right alignment.

Radio List — choose exactly one from a list (single-select). Same Items, Display mode, and Position options as Checkbox List.

Switch Field — a single on/off toggle (e.g. “Do you smoke?”).

  • Initial state — on or off by default.
  • Switch color — the accent color when on.

Yes / No (legacy) — Simple Yes/No selection (legacy). Prefer Radio List or Switch Field for new forms.

US State Picker — a dropdown pre-loaded with all 50 states + DC. No setup needed.

Date Picker — a calendar field.

  • Format — display format, e.g. MM/DD/YYYY.
  • Data Return Type — Date String from Date Format (stores formatted text) or JS-Date Object.
  • Single date vs range — one date, or a start–end range.
  • Min date / Max date — the earliest/latest selectable date.
  • Min days / Max days — for ranges, the allowed span.
  • Number of Months / Number of Columns — how many month panels show, and across how many columns.
  • First day of week — Sunday through Saturday.
  • Prefill with today’s date — drops in the current date (handy for “Date signed”). Applies only when a patient is associated with the link.

Signature Field — Collect Signature Online. The patient signs with a finger or mouse; the signed result renders into the PDF that files to Open Dental imaging.

  • Min width / Max width and Background color — these three settings currently have no effect on the form a patient sees.
  • To change the size of the signing box, set its Container Size Class instead — see Increasing size of Consent Signature box.

File Uploader — File Uploader Control. The patient attaches files or photos (insurance card, ID).

  • Allowed extensions — default .gif,.png,.jpg,.jpeg.
  • Max size — the per-file size limit.
  • Allow multiple / Maximum files — single vs several files, and a cap.
  • Button label — default Select File.
  • Validation — add the required rule to make the upload mandatory (e.g. a photo ID); min / max cap the number of files.

Open Dental Data Field — Add Allergies, Medications, Diseases / Problem, and Referrals with Options from your Open Dental. Pulls real lists from Open Dental so the patient picks from the practice’s actual data rather than free text.

  • Data type — DISEASES, ALLERGIES, MEDICATIONS, REFERRALS, or DISEASES with Category and Checkbox Options (loads the matching list for this site).
  • Option curation — after picking a type, the office chooses which options patients can pick from (Select Options you’d like user to pick from or Select All). For MEDICATIONS, all options load pre-selected and you deselect the ones you don’t want. REFERRALS works like Diseases: nothing is pre-selected — pick options or Select All.
  • DISEASES, ALLERGIES, and MEDICATIONS render as a multi-select and can include a None option; selections can write back to the chart. Only one field per form can save to each of Diseases, Allergies, Medications, or Referrals — see One field per form for each Open Dental list.
  • REFERRALS is different. It loads your practice’s referral sources from Open Dental — each shown as the referral’s last name and first name, excluding any that are hidden or that are themselves patients who referred someone else — and the office curates the list the same way as Diseases. For the patient it renders as a single-choice dropdown with no “None” option: they can pick at most one referral source, or leave it blank. With Save to Open Dental Field set to Referrals, their pick is attached to their chart as their referral source — added if they don’t already have one, or replacing the one they do. Leaving the question blank never removes an existing referral source. A control’s Data type and Save to Open Dental Field must agree: set Save to Open Dental Field to Referrals only when Data type is REFERRALS (and a Referrals control can’t be saved to any other Open Dental field) — otherwise the builder refuses Save / Save & Close with a message explaining why.
  • Alert highlighting — for DISEASES, ALLERGIES, and MEDICATIONS, a patient who selects only None never highlights, even with Highlight for Alert on; by default, any selection other than None highlights. REFERRALS has no None option, so with Highlight for Alert on, any pick highlights. To alert on specific options only (any data type), pick them in the “Highlight only when the patient selects any of:” picker that appears in this section once Highlight for Alert is checked.   • DISEASES with Category and Checkbox Options shows Diseases as tap-to-open category buttons with checkboxes instead of a dropdown, so patients group and tick conditions instead of searching one long list. Same Diseases chart write-back, same Highlight for Alert behavior. Full walkthrough: Health History by Category.

Attachment (legacy) — File attachment upload (legacy). Like File Uploader but also accepts PDF/DOC/DOCX. Supports the same required / min / max rules. Use File Uploader for new forms.

Label — a standalone piece of label text, optionally linked to another control.

Text Block — Block with text only (without any controls) — static instructions or disclaimers. Supports per-language translation.

Wiki — WYSIWYG Editor — a rich-text block with a full formatting toolbar (headings, lists, tables, and images) for longer formatted content like consent language. Edit it in the control’s Configuration panel. Images work just like the standalone Wiki — Upload File, drag-and-drop, paste, or Add URL (Teamio re-hosts its own copy so the picture won’t vanish later) — and they show for patients on the form and in the PDF that files to Open Dental. The image appears in the builder’s left-hand preview once you Save the Configuration panel; it doesn’t live-update while the panel is open. Supports translation. See Using the editor → Images.

Empty Block — Empty block to design your section/row — an invisible spacer for laying out controls side by side.

Page Break — Page break indicator for Printing PDF — marks where a new page starts in the printed PDF. No effect on the on-screen form.

Button — Simple button for your own purpose.

  • Button type — button, submit, or reset.
  • Run validation — whether clicking it validates the form first.
  • Emit event — an advanced hook to trigger custom behavior.

On choice controls (Dropdown, Checkbox List, Radio List), you can flag individual options as alerts so only a concerning answer highlights red. The per-option Flag as alert checkboxes are disabled until the control-level Highlight for Alert is checked. If you turn on Highlight for Alert but flag no specific options, any non-empty answer triggers the highlight. See Configuring a control > Highlight for Alert.

The Open Dental Data Field works the same way but with its own picker instead of per-option checkboxes: with Highlight for Alert on, expand Control Specific Configuration and use “Highlight only when the patient selects any of:” to pick specific allergies/diseases/medications that should trigger the highlight. Leave it empty and any selection other than None triggers it — None alone never does, no matter how the picker is set.

Whether a control highlights (with Highlight for Alert on, and no per-option/per-value narrowing) comes down to whether the patient gave a real answer:
